Parents, below there are two links to short forms for you to complete. Depending on which section of band your student is signed up for you need to be sure to complete the correct form (see images).

Info for students enrolled in the traditional band class:
If your student is enrolled in one of the traditional band classes, please click the button below to open the microsoft form. On the form you will enter your name, your student's name and an email. Once you submit the form, you will receive an email from me with additional information about the traditional band class including covid protocols and supplies.
Info for students enrolled in the MODERN BAND class:
